Keystone Advisors is looking for HIM Senior Clerks to support one of our healthcare clients in the Illinois Medical District in the West Loop of Chicago.

This position is Hybrid. 


POSITION SUMMARY
HIM Senior Clerks receives discharged medical record charts from Stroger Hospital, Provident Hospital, and ACHN daily via Building & Grounds Courier. Under the supervision of the HIM Supervisor/System Manager, Health Information Management, Senior Clerks must ensure all outpatient and inpatient records are processed daily to ensure completeness, accuracy, timely retrieval, and secure maintenance.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Prep and prepare discharged medical records for imaging.
  • Perform document imaging duties (prep, scan, quality control, and validation).
  • Perform computerized and electronic data entry and/or scan charts in accordance with department policies and procedures.
  • Batch, prepare, assemble, and sort records/reports for entry into hospital electronic database(s) and record(s).
  • Review documents ensuring correct patient labels are on the documents.
  • Analyze medical record for required content and signatures before scanning.
  • Verify the financial number is on each document and legible.
  • Prepare encounter cover page (ECP) when needed.
  • Ensure ACHN documents are scanned with appropriate scanning inclusion cover sheets.
  • Maintain files in a neat and orderly manner in accordance with departmental policies/procedures.
  • Meet departmental standards for quality and quantity of work.
  • Print patient barcode labels if missing from record.
  • Verify and combine duplicate MRNs paired by Healthe Intent software.
  • Incorporate continuity of care (CCD) documents into electronic medical records.
  • Send continuity of care (CCD) documents to outside institutions via direct messaging.
  • Performs other duties as assigned.

  • Attends departmental required in-service and training in-service programs as requested or required.
  • Attends the Hospital's annual in-service and training programs as requested or required. 
  • Adheres to section/departmental procedures for regarding work assignments.

  • Adheres to the Departmental Standard Practices Guidelines.
  • Adheres to Hospital policies on Attendance. 
  • Adheres to environmental requirements. 
  • Perform assignments as scheduled with or without supervision. 
  • Accepts changes in job duties, responsibilities, and job location as requested/required. 
  • Maintains the confidentiality of information.
  • Maintains a cooperative working relationship with coworkers and management.
MINIMUM REQUIREMENTS
Educational Requirements:
  • High School Diploma or GED Equivalent. Some college education in Health Information Management field is preferred but not required.
  • Recent Health Information Technology (HIT) student graduates encouraged to apply.

  • At least one to two years of experience in Health Information Management or healthcare.
​​​​​​​Area of Experience:
  • Medical Record Environment: Hospital, Ambulatory, Long Term Care, Physician Office.  
Specific skills or competencies needed to perform essential duties:
    • Computer Literacy
    • Data Entry Accuracy
    • Good Organizational and Communication Skills.
PREFERRED QUALI FICATIONS
  • Prior experience utilizing Cerner Electronic Health Record
  • Knowledge and proficiency in Microsoft Office Suite (Word and Excel).
  • Knowledge and proficiency utilizing medical terminology.
  • Knowledge and proficiency utilizing Cerner Power Chart EMR and Cerner Access Discern Database
  • Excellent verbal and written communication skills necessary to communicate with all levels of staff and a patient population composed of diverse cultures and age groups.
